Collecting Revenue Verification Documentation
The Revenue Verification Form is required annually for all new and renewing
participants. We recommend filing these forms in a secure cloud-based platform like
Google Drive and DropBox the same way your chapter keeps EO Member forms. Per
the 90-Day Action Plan (FY22-23), EOA Global will spot-check this documentation each
quarter to confirm they are on file.
Please note that the EOA version of this document intentionally does not include the
$250k-$999k requirement on it, as our goal is to simply have them confirm their actual
revenue. If participants do their bookkeeping and accounting in-house, the form
should be signed by the employee who actually does the accounting (not the
participant).
Although we can accept a chapter-specific form if it includes participant name,
revenue, and bookkeeping/accountant verification, we highly recommend
participants complete an EOA revenue verification form as a best-practice during the
onboarding and renewal processes.
Again, our main objective with this standard is for programmes to build this revenue
verification form piece into the onboarding and renewal processes—we are
specifically reviewing the RVFs for correct completion, not participant eligibility.
As a note, chapters protected by the GDPR are not required to submit this form.
